Wabash Community Health Center: Family Care Nurse Practitioner or Physician Assistant

Description

The Convenient Care NP or PA performs all functions for basic delivery of convenient care services. They will plan, implement, and evaluate care per supervisory agreement with physician. Provides acute and episodic care, provides primary preventative care and chronic disease management. Initiate and maintain accurate records, appropriate legal documents and other health and clinical reports. Provide services in accordance with the policies of the medical facility. Arrange for or refer patients to needed primary care services for establishment for chronic conditions that cannot be provided through convenient care clinic. Assure that adequate patient health records are maintained and transferred as required when patients are referred. Participate in periodic and joint evaluation of services rendered, including, but not limited to, chart reviews, patient evaluations, and case outcome statistics. Participate in the joint review and revision of adopted protocol for medical care plans.

Schedule: Monday – Friday    Hours: 12:00 pm – 8:00 pm

Requirements:

  • Graduate of an accredited school.
  • Licensed and current registration as a certified DEA Nurse Practitioner or Certified Physician Assistant in the State of Illinois.
  • Must meet any applicable state licensure standards.
  • CPR certification required.
  • Clinical experience preferred.
